If you are running Razor bars and you did not install the star nut deep enough into the taper it is probably possible for the star nut to start pulling out a little under pressure which would allow it to loosen so make sure your star night is secured good in there.

That could probably work but using a bottom race as a top cap won't be nearly as solid as a real threadless headset because those bottom races are a slightly bigger diameter than the steerer tube so it might be a little loose. Thread less fork = Thread less headset That's the whole point.
