Rad Bar Problem! HELP!!!!

I jus got rad bars and i put them on my MGP. I was tightening the clamp and when i finished, the bars slipped rite off. I tried this on my mgp fork, and my friends razor fork. What is the problem?!

The bars are standard 1/1-4 and yes they do have a slit
