For some reason hex bolts don't work well for HIC. But allen-head bolts do, so if it's a hex bolt (6 sided) ditch it Locktite the bolt threads, make sure you use blue locktite, red is permanent.

Yeah, use a tiny bit of locktite on the threads a few threads above where your headset top is. Use blue locktite, red is permanent. That's how my threaded setups stay nice and dialled
