Plus the form that you put your street address, name, etc. in looks kinda sketch. Notice how on most forms like that (except for this one) your web site address bar at the top of your browser has a lock icon on it and goes a different colour? It doesn't do that on this one, which I think means that it is unsecured and almost anyone can use and look at the info that you've put in there..

I checked it out and yeah..its not a secure website hahah so pretty much anyone (who knows what they are doing) can look into all of the information typed into that website =[

So what is someone going to do with your name and address... send you some mail, knock on your door, try to break in your house? That stuff can happen if you don't fill out the form, its a legit business anyway, people allready got their grips.
